NOTICE OF RIGHT TO COPY OF APPRAISAL

This notice is being provided to you pursuant to 12 CFR § 1002.14(a).

We may order an appraisal to determine the property’s value and charge you for this appraisal. We will promptly give you a copy of any appraisal, even if your loan does not close. You can pay for an additional appraisal for your own use at your own cost. You will be provided a copy of each appraisal or written valuation concerning this property promptly upon completion, or three (3) business days prior to the time you become contractually obligated on the transaction (for closed-end credit) or account opening (for open-end credit), whichever is earlier.

 

VOLUNTARY SUBMISSION OF VERIFICATION INFORMATION

Applicant(s) understand(s) that North Shore Trust and Savings cannot require the submission of verification documents (such as tax returns, pay stubs, W-2 Forms or bank statements) until I/We have received a Loan Estimate and indicated an intent to proceed. In order to expedite my/our request for a loan preapproval without an identified property address, I/We wish to proceed with the pre-approval request and voluntarily consent to providing any and all verification documentation required by North Shore Trust and Savings to process our pre-approval request and issue a determination. I/We understand that we will not receive Loan Estimate until a property address has been identified.
